Youth Fellowship Camp @ Prairie Camp for 12-15 year olds

*Camp is for youth from the ages of 12-15. As long as they turn 12 on or before July 10th of this year, they may register for their first year of camp.
You will be enrolled in 2 activity classes each day. Your first activity choice and second activity choice will let us know which two are your top pick. If we are not able to put you into both of your top two choices, your 3rd and 4th activity choices will let us know which of the other activities you would also enjoy.
This class will be making a marshmallow gun and then having a marshmallow war at the end of the week.

I do hereby give my consent for any necessary medical treatment to be rendered to the above named minor while they are at Youth Fellowship Camp located at Prairie Camp (63526 CR 9 Goshen IN 46526). Medical treatment may include emergency examination, anesthetic, medical diagnosis, minor surgery or treatment, and/or care. This will also be rendered under the general or special supervision and on the advice of any physician licensed to practice medicine. I understand this is to be used only if I cannot be reached. I understand this is to allow emergency treatment to be initiated without delay and the staff will continue efforts to contact me. Secondary medical insurance is covered in the cost of registration. I further recognize that certain hazards and dangers are a possibility at camp events. I understand also, that although the camp has taken precautions to provide proper supervision and instruction for each activity, it is impossible for the camp to guarantee absolute safety. Further, I waive any claim that may arise against Youth Fellowship Camp, Griner Church, Prairie Camp, The Missionary Church, North Central District, Inc. and/or employees or volunteers, as a result of participation in the program, except for those that are a direct result of gross negligence of the camp or its employees and/or volunteers. I also give permission as a parent or guardian for my child to be photographed or videotaped for promotional purposes of Youth Fellowship Camp. I am also aware of the Youth Fellowship Camp Guidelines and my child is willing to follow the standard of the dress code.

As the YFC team, our heart is for camp to be affordable for every family. We are so grateful for the generosity of our sponsors, who make this low price an option that we can offer. The minimum fee is $100, but the actual cost per camper in recent years has been $250+. If you would like to pay above the $100 fee (the amount is up to you), this greatly blesses camp and helps to cover more of its operating expenses.
*Full refunds can be issued up until June 1st. After June 1st, if you withdraw your camper, refunds will not be issued due to the funds being tied up in camp expenses.
Money for the snack shop is separate from the registration fee. You can pick the amount you would like to add to your camper's account for the week. Snack shop is typically open 2 times each day. Any money that is leftover from their account at the end of the week will not be refunded. We try to keep our snack shop prices at cost or just a bit above per snack, candy, and drink that we buy in bulk. $20 - $30 is a pretty common amount for campers to have and spend.
